Litchfield Home Care and Hospice seeks a Social Service Coordinator to join its team. Litchfield Home Care provides in-home and community based services in the Minnesota Litchfield area. Ecumen creates home for older adults wherever they choose to live. This position is with our Home Care and Hospice facility and the shift is days and on-call. The Social Service Coordinator provides psychosocial assessments and referrals as appropriate to the client and family who has been diagnosed with a terminal illness. Specific responsibilities include:
Provide assessments of the social, emotional and financial needs of the client
Assist clients and families in accessing appropriate services
Facilitates client conferences
Participates in hospice orientation and training

This position requires a person who has a Masters Degree in Social Work. Related work experience is also preferred. All Ecumen employees must possess the ability to communicate effectively, including written and verbal skills; work well, both individually and as part of a team, through respect, collaboration and dedication; and successfully interact with a diverse culture base. The successful candidate must be able to prioritize and organize work effectively and efficiently, and demonstrate a positive attitude, high integrity and ethical awareness.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
